Terms and conditions of participation in the Private Health Services pilot

These terms and conditions are intended to inform you of your participation in the Private Health Services pilot. This pilot is an initiative of ONVZ Zorgverzekeraar N.V., with its registered office in Houten and trading under the name Private Health Services (hereinafter: ‘ONVZ’ or ‘Private Health Services’). As a participant (hereinafter: ‘Participant’) you agree to the provisions included in these pilot terms and conditions. 

Participation in this pilot will not affect your health-care plan agreement and does not serve as a replacement for regular medical and other care. The pilot is intended for testing and evaluation purposes. These services are not intended as an alternative for qualified health-care providers; in an emergency you must always contact the emergency services by calling 112.

1. Goal of the pilot

The pilot is intended to test and evaluate the services provided by Private Health Services in terms of their quality, relevance and feasibility. The pilot focuses on personal health support and access to top-rated health care. 

2. Duration of participation

Participation starts at the time of registration and runs until 1 October 2026 at the latest, unless ended sooner by ONVZ or the Participant. ONVZ reserves the right to stop the pilot early, subject to a notice period of one month. ONVZ can end participation with immediate effect for compelling reasons, which include fraud, misuse of services, or if ONVZ cannot reasonably be expected to continue the participation. 

3. Conditions and obligations

  • The Participant will be given access free of charge to the services of Private Health Services as described in Article 4.
  • The Participant is responsible for providing correct and current information.
  • Participation is personal and non-transferable.
  • The Participant can end their participation at any time by contacting Private Health Services through the contact page
  • If the Participant provides incorrect information or uses the services in breach of these terms and conditions, this may result in their participation being terminated.

4. Services 

A. Product

During this pilot, ONVZ will give the Participant access to Private Health Services. 

B. Services

As part of the pilot, Private Health Services will provide the following services

  • Access to consultations with a Personal Health advisor, your first point of contact for everything involving Private Health Services
  • ‘Your gateway to the very best health care’: we support the process of choosing a health-care provider
  • An account on the Private Health Services platform, giving you insight into your own data, appointments with your Private Health advisor and health-care providers, plus possible treatment programme costs
  • Facilitating access to selected health check-ups
  • Support for access to selected mobility, accommodation and support services such as drivers
  • Additional services in the field of health and health care, which will be gradually expanded during the pilot

The current range of services is always available from your Personal Health advisor or on our website. 

C. Availability of services

During the pilot, Private Health Services cannot guarantee the availability of all services. 

D. Use of third parties

Some of the services provided as part of this pilot will be provided by third parties, such as suppliers, partners or subcontractors. Private Health Services will always inform you about this. 

E.  No third-party liability

Private Health Services cannot accept liability for any loss whatsoever resulting from the acts or omissions of these third parties. Any claims or demands from the Participant in relation to the services provided by third parties must be submitted directly to the third party in question.

5. Payment conditions for third-party services

If the Participant wishes to use third-party services that are subject to a fee, these costs will be clearly communicated beforehand and only charged following the Participant’s explicit agreement. Payment will be made directly to Private Health Services, unless agreed otherwise. 

6. Privacy and data processing 

ONVZ processes personal data in accordance with applicable privacy legislation and ONVZ’s privacy statement. Medical data will only be processed following the Participant’s explicit permission. 7. Liability

ONVZ cannot accept liability for any loss arising from the use of the pilot services, unless it involves a deliberate act or gross negligence. The Participant will remain personally responsible for medical decisions and must always contact a doctor when in doubt.

8. Termination of participation

Participation will end:

  • At the end of the pilot period
  • If the pilot period is stopped early by ONVZ
  • If the Participant cancels their participation
  • If ONVZ ends participation due to fraud or death

9. Complaints procedure 

If the Participant has a complaint about the pilot as a whole, any part of it or its performance, this complaint can be submitted in writing to the point of contact designated for the pilot. ONVZ will respond to the complaint as soon as possible, but no later than four weeks after submission

10. Applicable law

This agreement is governed by Dutch Law. Disputes will be submitted to the competent court in Utrecht.

11. Changes to terms and conditions

ONVZ reserves the right to change these terms and conditions during the term of the pilot. The Participant will be informed electronically and in a timely manner about any changes. If the Participant does not agree to the changed terms and conditions, they will be entitled to end their participation in the pilot with immediate effect.

12. Intellectual property

All intellectual property rights regarding the services, systems, documentation, materials, data and other information offered as part of the pilot are exclusively held by ONVZ or its licensors. The Participant is expressly forbidden from removing or changing any intellectual property designation, or using its content for commercial purposes or for any purpose other than that for which it is provided without ONVZ’s prior written permission. Any infringement of ONVZ’s intellectual property rights will lead to immediate termination of participation in the pilot and may also result in legal action.
